As Community Health Workers, our specialists provide education, information, advocacy and resources that improve or manage the health and wellbeing and quality of life of individuals in the community with disabilities, chronic health conditions, home or food insecurities that can lead to worse health outcomes. Our Community Health Specialists are trained and educated in a various aspects of holistic health and wellbeing, and our main goal is to improve and enhance the health outcomes of all individuals we work with.
